In the Management module of QC the Test Manager will define a folder for Releases under which the Cycles are created. With the help of this feature in QC, we can manage application releases and cycles more efficiently, we can track the progress of an application release against your plan to determine whether the release is on track.

Releases and cycles of each release can be defined under the management module, mostly by developer or manager.
Then when requirement specifications are defined they will be assigned to any release and cycle.
In the test lab when test execution is done and when defect is found defects will also be assigned where the defect is found.
That is the how QC maintains releases and cycles.
